We are seeking a skilled and experienced travel Physical Therapist to join our team in Demorest, Georgia. As a travel Physical Therapist, you will provide high-quality patient care and work closely with our multidisciplinary team to deliver exceptional outcomes.
**Key Responsibilities:**
- Provide physical therapy services to patients in their homes, as well as in-office visits
- Develop and implement individualized treatment plans to meet the unique needs of each patient
- Conduct thorough assessments and evaluations to identify areas of improvement
- Collaborate with other healthcare professionals to ensure seamless care coordination
- Document patient progress and maintain accurate records
**Requirements:**
- Minimum 1 year of adult home health experience
- Recent home health experience required
- Ability to travel up to 75 miles from home clinic
- Weekend availability varies per location
