**Responsibilities**
- Provide patient care in a home setting.
- Possess and demonstrate excellent communication skills, leadership skills, organizational skills, and customer service techniques
- Perform and manage multiple responsibilities concurrently and work well under pressure
- Responsible for supervising services provided to client’s and determine which clients are appropriate.
- Participate in the development and review of each client's service plan.
- Maintain accurate and detailed medical documentation in accordance with HIPAA regulations.
- Collaborate with physicians, healthcare professionals, and other team members to develop and implement treatment plans.
- Educate patients and their families on health and wellness practices, disease prevention, and treatment options.
- Participate in quality improvement initiatives to continually improve patient outcomes and reduce readmissions.
- Perform supervisory home visits to each client's residence at least every 62 days for skilled nursing services.
- Perform supervisory home visits to each client's residence at least every 92 days and at least and occasionally visit while the PCA is present and working.
- Perform supervisory home visits to each client's residence at least every 122 days and at least and occasionally visit while the companion and sitter is present and working.
- Required to annually assess and supervise the performance of the employees through direct observation and demonstration of the tasks that the employees are assigned to perform.
**Qualifications**
- Current licensure as a Registered Nurse in the state of practice.
- Strong knowledge of diagnostic evaluation
- Familiarity with aseptic technique and adherence to infection control guidelines.
- Experience with HIPAA regulations and medical documentation practices.
- Strong communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment and prioritize patient needs.
- Commitment to providing patient-centered care and supporting the healthcare team.
**Skills**
- Basic life support certification.
- Proficiency in diagnostic evaluation
- Knowledge of aseptic technique and infection control guidelines.
- Experience providing home health care.
- Familiarity with HIPAA regulations.
- Strong medical documentation skills.
