What are the responsibilities and job description for the Admissions Coordinator RN or LPN position at Pleasant View Home?
Position Purpose:
The primary purpose of the Admissions Coordinator is to coordinate and manage all resident admissions in long term care. The Admissions Coordinator will collaborate directly with residents, families, and PVH staff to ensure a smooth transition for all residents to Pleasant View Home.
Supervision:
The Primary Care Nurse is under the direct supervision of the facilitys Director of Nursing and/or the Assistant Director of Nursing as delegated by the Director of Nursing.
Qualifications and Position Requirements:
- The desired minimum requirements necessary to effectively perform position responsibilities include a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN/RN program; excellent leadership skills or an acceptable equivalent combination of education and experience, and respect for the principles of resident rights, confidentiality, EEO and ADA
- Must possess a current, unencumbered, active license to practice as an RN or LPN in Kansas
- Must be able to read, write, speak, and understand the English language.
- Must possess the ability to make independent decisions when circumstances warrant such action.
- Must possess the ability to deal tactfully with personnel, elders, family members, visitors, government agencies/personnel, and the public.
- Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
- Must possess a willingness to work harmoniously with professional and non-professional personnel.
- Process all daily resident referrals in collaboration with the Director of Nursing (DON) and Administrator.
- Oversee the coordination of pre-admission screenings, admissions, transfers and dismissals and work with other departments to ensure successful transfer of residents.
- Oversee discharge planning upon admission, ongoing and discharge. Complete discharge summary, if necessary.
- Ensure that incoming residents are appropriately placed in terms of the care needed.
- Evaluate resident care according to new residents physician orders and other needs.
- Determine incoming residents level of independence and choices consistent with their care plan.
- Confer with residents and/or family about resident's cares and concerns as appropriate.
- Ensure completion of admission assessments on new residents.
- Review various healthcare audits as directed by the DON, Assistant DON (ADON), or Administrator.
- Support the MDS Coordinator by conducting assessments.
- When time allows, or as directed by the DON or ADON, to perform any nursing duties to fulfill residents individual care plan.
- Support the DON and ADON with documentation of staffing issues.
- Experience in long-term care and/or management experience is required.
- Possess proficient assessment skills related to geriatric residents.
- Must be able to read, write, speak, and understand the English language.
- Must possess the ability to communicate with people at various levels of comprehension.
- Ability to develop good relationships with residents and their families.
- Operational understanding of all regulations pertaining to Skilled Long-Term Care.
- Ability to establish and maintain effective working relationships within the nursing department and with other departments within the facility.
- You must be willing and able to travel in the surrounding area to complete in person admission assessments.
- Constant collaboration with the marketing director to recruit and market new residents for our long term care facility.
