We are seeking a dedicated and compassionate Nurse Practitioner to join our healthcare team. Duties
- Conduct comprehensive patient assessments and develop individualized care plans.
- Administer medications and monitor patient responses to treatments.
- Provide care in various settings, including hospice palliative medicine, home care, and acute care environments.
- Perform procedures such as catheter care, spinal taps, and laboratory tests as needed.
- Collaborate with interdisciplinary teams to coordinate patient care and enhance treatment outcomes.
- Educate patients and families about health management, medications, and treatment plans.
- Maintain accurate medical records and documentation in compliance with healthcare regulations.
- Advocate for patients' needs and work towards improving their overall well-being.
- Podiatric, wound care, and aesthetics.
- Hospital, nursing/assisted living/ rehabilitation facilities/ and home services availability
Requirements
- Master’s degree in Nursing or related field with a focus on Nurse Practitioner training.
- Current Nurse Practitioner certification and state licensure to practice.
- Experience in hospice palliative medicine, acute care, pediatrics, or home care is preferred.
- Proficiency in medication administration and familiarity with laboratory procedures.
- Strong interpersonal skills with the ability to work effectively with individuals from diverse backgrounds, including those with disabilities.
- Excellent communication skills for educating patients and collaborating with healthcare teams.
- Ability to perform physical tasks associated with patient care, including lifting and moving patients when necessary.
