Overview
We are seeking a dedicated and compassionate Nurse Practitioner to join our healthcare team. The ideal candidate will possess a strong background in patient care, with experience in various medical settings. This role requires a commitment to providing high-quality healthcare services, focusing on patient-centered care across diverse populations, including those with disabilities and chronic conditions.

Duties

  • Conduct comprehensive patient assessments and develop individualized care plans.
  • Administer medications and monitor patient responses to treatments.
  • Provide care in various settings, including hospice palliative medicine, home care, and acute care environments.
  • Perform procedures such as catheter care, spinal taps, and laboratory tests as needed.
  • Collaborate with interdisciplinary teams to coordinate patient care and enhance treatment outcomes.
  • Educate patients and families about health management, medications, and treatment plans.
  • Maintain accurate medical records and documentation in compliance with healthcare regulations.
  • Advocate for patients' needs and work towards improving their overall well-being.
  • Podiatric, wound care, and aesthetics.
  • Hospital, nursing/assisted living/ rehabilitation facilities/ and home services availability

Requirements

  • Master’s degree in Nursing or related field with a focus on Nurse Practitioner training.
  • Current Nurse Practitioner certification and state licensure to practice.
  • Experience in hospice palliative medicine, acute care, pediatrics, or home care is preferred.
  • Proficiency in medication administration and familiarity with laboratory procedures.
  • Strong interpersonal skills with the ability to work effectively with individuals from diverse backgrounds, including those with disabilities.
  • Excellent communication skills for educating patients and collaborating with healthcare teams.
  • Ability to perform physical tasks associated with patient care, including lifting and moving patients when necessary.

Job Types: Full-time, Part-time

Pay: $100,000.00 - $120,000.00 per year

Expected hours: 40 per week
