A Non Profit organization is looking to hire an Intake Coordinator. The candidate will be the intake coordinator and supervisor of the HCBS program., Applicant will be the first point of contact for individuals and families seeking services. The candidate will also supervise a team of coordinators, providing guidance and oversight to ensure efficient and effective service delivery.
Responsibilities:
Conduct initial interviews and assessments with individuals and families interested in HCBS services
Gather essential information such as demographics, needs, and preferences
Explain HCBS program eligibility requirements and service options
Guide individuals and families through the application process, providing clear instructions and support
Verify eligibility for HCBS services based on program guidelines
Maintain accurate and complete records of all interactions and applications
Schedule appointments for further assessments or service planning meetings
Collaborate with other team members to ensure a seamless intake process
Provide exceptional customer service, addressing questions and concerns with courtesy and empathy
Participate in ongoing training to stay current with HCBS program regulations and best practices
Delegating to coordinators

Qualifications: (some or most)
Master’s degree, or at minimum, bachelor’s degree in social work, human services, healthcare administration, or a related field
Minimum of 2 year of experience working with individuals with disabilities or in a related field
Strong communication and interpersonal skills, with the ability to build rapport with diverse populations
Excellent organizational skills and attention to detail
Proficiency in Microsoft Office Suite
Ability to work independently and as part of a team
Experience working with HCBS programs or other community-based services
Familiarity with disability rights and advocacy
Bilingual skills
