Hours: Monday - Friday, 9am - 5pm

Reports to: Director of Family Services

Salary: $47,000-$50,000

Job Summary

This position is responsible for building competencies and healthy outcomes for a caseload of approximately 25 African American youth 11-17 years old and their families over the course of six months, for a total of 50 youth served annually providing gang prevention services by performing the following duties.

Essential Duties

  • Assesses and assertively engages with youth using the Family Assessment Tool and implements strength-based individual success plans built on culturally specific and evidenced based practices to help participants develop and reach goals.

  • Develops and leads monthly service projects with youth to develop civic engagement and leadership skills, build positive peer connections and foster empathy and compassion.

  • Works collaboratively with other professionals in the field including schools, community-based organizations, Mental Health/ Alcohol and Drug organizations, Child Welfare, Department of Community Justice, and Oregon Youth Authority.

  • Uses creativity and resourcefulness to ensure that youth and their families are aware of all community resources available for them and advocate with other service providers for the needs of families as appropriate, which may include, but are not limited to, housing stability, counseling services, and prosocial activities.

  • Provides day-to-day activities that may involve social casework, intake, assessment, assertive engagement, and skill development through engaging youth and families in activities through: Education/Employment, Home/Family, Health, Pro-Social Activities, Spirituality, and Community.

  • Facilitates, participates, develops and coordinates pro-social activities for youth and their families.

  • Adheres to documentation requirements as indicated in CHI Program policies, including but not limited to accurate data entry into CHI Data base, case files, service plans, weekly/monthly reports, and case notes.

  • Respond to all crises and risk incidents within the required time frames per Portland OIC policies.

  • Participates in regular supervision check-ins, department staff meetings, community partner meetings and all staff trainings.

  • Fosters a safe, inclusive, and supportive environment that encourages participation and personal development.

  • Supports youth develop conflict resolution, communication, and decision-making skills.

  • Provides one-on-one coaching and mentoring to address personal challenges, school performance, and career aspirations.

  • Advocates for youth needs and contributes to program development based on feedback and best practices.

  • Tracks and assesses individual youth progress, adjusting strategies and support as needed.

  • Performs other duties as assigned.

Experience/Education

A bachelor’s degree in counseling or similar subject, or an associates degree with four years of equivalent experience working with vulnerable youth is required this role. Having a year of experience of gang-related work is preferred.
