Intake Coordinator

Reports to : Director of Intake

  • Payrate $22-$27 hourly

What you will do :

  • Process incoming referrals to the agency on multiple referral platforms
  • Guide referral sources in obtaining necessary documentation
  • Enter protected health information and understand insurance guidelines
  • Communicate directly with clients, provider’s offices and referral sources to coordinate care
  • Data entry
  • Provide excellent customer service throughout the referral process
  • Work as an integral part of a collaborative team

    • Ideal Candidate :

  • Minimum of 2 years experience in home care, skilled nursing facility, long term care, or acute care
  • Strong written and verbal communication skills
  • Ability to work in a fast paced environment
  • Understanding of medical terminology
  • Excellent customer service skills
  • Goal oriented
  • Good understanding of EMR, excel, Microsoft Office
  • Organizational and time management skills
  • Experience in a medical office setting
  • Typing skills
