Essential Duties and Responsibilities
Conduct comprehensive health assessments of residents, including physical examinations, medical histories, and medication reconciliation.
Develop and implement individualized care plans in collaboration with the interdisciplinary team, considering the unique needs and preferences of each resident.
Manage chronic conditions and acute illnesses, providing timely interventions and appropriate referrals as needed.
Order and interpret diagnostic tests, such as laboratory tests and imaging studies, to aid in diagnosis and treatment planning.
Prescribe medications and treatments within the scope of practice and in accordance with state regulations and facility policies.
Participate in care initiation for our chronic care management services.
Provide education and counseling to residents and their families regarding health maintenance, disease prevention, and self-care strategies.
Participate in quality improvement initiatives and interdisciplinary meetings to enhance the overall quality of care and resident outcomes.
Maintain accurate and up-to-date documentation of all assessments, interventions, and communications in the electronic medical record system.
Requirements
Master’s degree in Nursing and graduate of an accredited Nurse Practitioner program.
Unrestricted New York State Nurse Practitioner license.
Unrestricted Drug Enforcement Administration (DEA) permit to prescribe all medications commonly prescribed in the course of the practice.
Eligible to provide reimbursable services and maintain provider status with Medicare and Medicaid.
Preferred Experience
A minimum of 3 years of clinical experience, preferably in long-term care.
Experience with eClinicalWorks (eCW) electronic health records.
