Position Summary : This position is to be filled with a Registered Nurse who is approved by the Director of Nursing to provide clinical oversight to all Polaris Assisted Livings in the assigned region including the Lead RN in the building. Responsibilities include but are not limited to the completion of assigned assessments and required documentation, auditing records participating in the interviewing and training process for new and existing clinical staff, and participating as an active member of the CQI committee.
Qualifications Requirements : To perform this job successfully an individual must be able to perform each essential duty satisfactorily. Must be a Licensed Registered Nurse, and have 2 years experience as a professional nurse with administrative / supervisor experience in health care. Knowledge of the UAS and experience is required. The requirements listed below are representative of the knowledge, skill, and / or ability required. In addition, maintaining a professional and positive demeanor at all times is essential.
Reasoning Ability : Must have ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form. Ability to effectively present information in one-on-one and small group situations to clients, their families and other employees of the organization.
Other Skills and Abilities : Intermediate computer skills required Microsoft word and Excel must. The ability to utilize computer software integral to agency operations after initial training period and multi-tasking capability is a must.

Duties and Responsibilities :
Responsible for assigned UAS assessments for both the ALP / LHCSA within the region and submission of all required documentation within required time frame. Assessments will be assigned by the Director of Nursing or their designee.
Management of the Lead RNs in your region, oversight of resident s ervices rendered within your homes, in conjunction with the Lead RN and DON and in accordance with agency policy / procedure and state and federal regulations .
Regional Nurse only reassessment
Work in conjunction with the Lead RN / Executive Director to assure residents are at the appropriate level of care.
Knowledge of each home’s census in relation to admissions, discharges, and out of house residents in the assigned region.
Through Clinical Quality Audits assures completeness and compliance with all resident files in your region.
Ensure chart audit results are submitted to the DON timely for CQI deadline.
Works with DON to oversee and evaluate all quality assurance reporting such as incident / accident reports, infection control reports, and complaints / grievances for your region.
Ensure chart auditing is being completed by the Lead RNs and assisting when needed to ensure DON receives timely information for the QI meetings.
Consult with the DON in matters relating to quality improvement and client care services. Observe, assess and report any adverse sign(s) and / or symptom(s).
Meet regularly with DON in regards to quality measures, client care services, and reports any concerns timely.
Maintain strict client confidentiality.
Case conference with clinical staff and extended care team as needed.
Notifies DON immediately of any Department of Health communication or visits to the homes in the region.
Participate in peer review process as needed.
Participates in local and state organizations and activities relative to the health professions and community health services.
Supervises training and updates clinical staff on all Policies & Procedures, forms, DOH regulations, etc. as directed by DON.
Assist the DON with conducting performance evaluations for the Lead RN in assigned region as scheduled.
Conduct nursing probationary evaluation s and supervisions for lead RN’s in conjunction with the DON.
Assist in the interview and training of clinical staff as needed.
Attend office meetings via in-person or Zoom.
Obtain, if not already in possession of, additional certifications and participate in additional trainings, which include UAS, OASIS training, PRI / Screen Certification, and / or training in use of a QAS analyzer.
Other roles as assigned by DON on a particular day, which may include covering the lead RN role in an assigned home.
