POSITION SUMMARY: The Community Outreach Coordinator coordinates, attends, and reports on community events intended to promote HealthVisions programming and initiatives. The Community Outreach Coordinator organizes and coordinates the HVM Community Pantry and oversees referral processes, working with associates and community partners. The Coordinator strives to exemplify the mission, vision, and values of HealthVisions Midwest, Inc. and the Poor Handmaids of Jesus Christ (PHJC).
PRINCIPAL RESPONSIBILITIES AND DUTIES:
- Coordinate HealthVisions outreach events such as community resource/health fairs, networking events, and community forums. Identify and attend community events as an HVM representative to promote programming and services.
- Responsible for promoting programs and services, developing workshops, recruiting participants, and organizing events based on the community's needs and HealthVisions’ mission.
- Schedule and conduct presentations about HealthVisions and articulate the organization’s mission to establish strong partnerships with other community organizations that expand services.
- Manage HVM’s Community Pantry Program. Oversee all databases, donations, invoices, and program tracking. Purchase and inventory all items for the pantry in adherence to annual budget. Continually improve the pantry, as needed, based on the needs identified by the community and HVM stakeholders.
- Coordinate HealthVisions Midwest’s Lake County Health Equity Coalition. Schedule monthly meetings, create agendas, and assist coalition members in identifying goals and priorities. Recruit and maintain engagement with coalition members.
- Manage the HealthVisions Midwest referral process to ensure clients access the best services and resources in the community. Determine eligibility to receive services and priority of need based on information provided by community members.
- Supervise, train, and equip support outreach staff when/if support positions are filled. Determine staff schedules, review and answer PTO requests, and provide other support as needed by staff.
- Develop and manage community event calendar and ensure registration and employee coverage for events. Build partnerships with other providers to serve as resources for our staff and clients.
- Maintain budget and inventory for outreach materials and supplies, complete purchases as necessary, and ensure outreach materials are stored and organized.
- Facilitate training and educational sessions for Community Health Workers, partner organizations, and the community to promote healthy living, mental health awareness, and other related topics.
- Oversee the development and distribution of the HealthVisions Hammond quarterly newsletters and community correspondence. Ensure all marketing materials are high quality and consistent with the HealthVisions brand.
- Create marketing and recruitment opportunities to attract program participants by scheduling time at local businesses, hospitals, clinics, etc.
- Collaborate with the staff and PHJC Communications to develop and manage all marketing materials of various types of media such as, but not limited to, print, radio, TV, internet/web-based social media, etc.
- Serve as a liaison between the community and HealthVisions by attending community meetings as a HealthVisions representative. Update the Director of community happenings as necessary.
- Continually strive to exemplify the mission and values of our sponsors and respect the confidentiality of the organization. Provide a warm introduction to the program for those who inquire.
- Perform other duties as assigned to meet the vision, philosophy, mission, and values of HealthVisions Midwest and the Poor Handmaids of Jesus Christ.
MINIMUM QUALIFICATIONS:
- The above knowledge, skills, and abilities are normally acquired through completion of a bachelor's degree in a related area, or an equivalent level of knowledge obtained through a combination of education and experience.
- Ability and willingness to work some evenings and weekends when required.
- Public speaking skills, training, and previous supervisory experience are helpful. Knowledge of public health, community health, and community health workers is required.
- Certified Community Health Worker Training (within 6 months of hire).
- Type at least 45 wpm.
- Valid driver's license and vehicle for driving with car insurance and good driving record.
REQUIRED SKILLS:
- Microsoft Access, Word, Excel, PowerPoint, and Outlook; typing and computer skills are essential.
- Previous experience in leading and supervising individuals and teams.
- Strong oral and written communication and presentation skills.
- Ability to work with sensitive information, including HIPPA laws.
- Ability to work independently and in a team environment.
- Ability to manage multiple projects with attention to detail.
- Initiative, the ability to work independently to plan, organize and set priorities.

Salary : $48,000 - $53,000
