POSITION SUMMARY
Develop, establish and maintain relationships with community referral sources and stakeholders in order to develop and implement Porter Starke Service’s crisis continuum, including crisis receiving and stabilization services. Plan and / or attend community events to increase community awareness and to present a positive image for the facility. The role serves as a liaison between the facility, referral sources, clients and the community to consistently provide a high level of customer service. Facilitate linkage and relationships with appropriate service agencies and facilities to assist staff with the most appropriate resources for clients.
VI ITAL DUTIES AND RESPONSIBILITIES
Provides services to ensure / increase access to appropriate care
Actively seek to identify new potential referral sources and maintain on-going relationships to improve efficacy and outcomes of the client’s needs.
Assist in ensuring all referrals are well-coordinated and have successful outcomes.
Routinely participate in relevant community organized meeting and programs.
Participate in the development and execution of special events as assigned.
Keep referral sources informed of program changes within the facility.
Consistently build community awareness and ensure a positive image of the facility.
Facilitate and maintain client resources for primary / specialty healthcare, communication amongst facilities and stakeholders and provide appropriate referral sources to services outside the agency.
Facilitates identification of high-risk or at-risk populations and take action to discover and engage referral sources to facilitate access to care and resources.
Performs other duties as assigned.

QUALIFICATIONS :
Bachelor Degree in Psychology or Social work is required.
One year of mental health experience is preferred.

CERTIFICATION / LICENSE
Must possess a valid driver’s license.
Must be CPR certified (training provided)
