At PHC our team of Nurse Practitioners are the core of our proven primary care model. Acting independently as primary care providers, they provide high quality, high touch primary care at the patients home or residential setting. The ideal candidate is passionate about caring for vulnerable populations and is able to implement innovative care solutions to address complex needs. Our Nurse Practitioners are fully supported by and integrated care team.
Position Essential Functions:
- Delivers high-quality, cost-effective primary care to assigned patients
- Completes comprehensive histories and physical exams, develops assessments/diagnoses and creates, executes and evaluates treatment plans
- Addresses advanced care planning, social determinants of health and preventative measures by engaging holistically with their patients for the best possible outcomes
- Collaborates with the Care team in the development of an integrated care plan that identifies key patient problems, goals of care, barriers to meeting goals, interventions, activation level and patient self-management activities
- Strong communication skills, excellent time management and organizational skills as well as a self-started are essential
- Participates in grand rounds, panel rounds and multi-disciplinary team meetings
Qualifications
Position Requirements:
- Holds active unencumbered license in Arizona
- Board certified with AANP or AANC
- Has an active DEA license
- Current CPR certification
- Two or more years of practice experience preferred
- Must have valid state driver license and automobile insurance
