What are the responsibilities and job description for the MDS Coordinator (LPN) position at Port Saint Lucie Rehab and Healthcare?
Our skilled nursing facility located in Port Saint Lucie, FL is now recruiting an experienced full time MDS Coordinator-LPN to perform all aspects of MDS monitoring, including the following:
- Assessing patients and collecting data for the Minimum Data Set (MDS); timely completion of accurate assessments and interdisciplinary care plans that comply with Federal and State guidelines and our organization's policies and procedures.
- Collating charts and chart data, inspecting records, and creating reports.
- Implementing and explaining federal and organizational regulations and guidelines.
- Maintaining patient medical records and developing individualized care plans with targeted outcomes.
- Must have knowledge in the RAI systems, OBRA related program and PPS
- Perform other duties as assigned.
Job Requirements
- FL LPN License with at least three years experience working within a long term care environment as part of the MDS Department, preferably a SNF.
- Ability to work collaboratively, as part of the clinical team, management and other staff members to accomplish goals of the department.
- Excellent interpersonal and communication skills required.
