What are the responsibilities and job description for the Next Memphis Family Liaison position at Porter-Leath?
Under the direction of the NEXT Memphis Family Service/Health Supervisor, the Family Service Worker will provide social work/case management and health services to children in the Preschool program and their families; will create, carry out and monitor the activities designed to implement best practices to promote parent-child engagement.
Requirements:- Bachelor's degree in social services or a related field is required.
- 1-2 years of experience in a social service or educational setting
Supervisory Responsibilities
This position does not include any supervisory responsibilities.
Essential Job Functions
- Perform according to the standards set forth by the Tennessee Department of Education, Tennessee Department of Human Services- Child Care Licensing, Porter-Leath, and NAEYC.
- Assist with completion of the application/enrollment process with participating families.
- Maintain family/child records and documentation in the Database system.
- Assist with providing extensive case management services required by families involved in Next Memphis to include, ensuring that enrollees have access to needed services.
- Assure all clients enrolled have access to medical care. Develop and implement follow-up plans to ensure medical care is obtained and maintained according to Next Memphis regulations.
- Work with parents to promote parent participation in the delivery of services for their children.
- Assist with recruitment of families with children ages six (6) weeks to five (5)years old.
- Provide information to the community about the program to recruit for eligible families and maintain enrollment.
- Assists with the completion of the Family Partnership Agreement and ensures regular follow-up to help families achieve goals as outlined in the Parent, Family and Community Engagement Framework.
- Completion of professional, accurate and thorough family contact notes in the Database system.
- Coordinate and assist parents in planning monthly parent meetings and activities.
- Assist participants in locating/enrolling in life skills training classes/workshops that will meet their particular needs in the areas of: job readiness, career development, budgeting, credit counseling, conflict resolution, leadership skills, community advocacy, GED, etc.
- Assist with the coordination or health screenings required for Next Memphis participants.
- Assist with the completion of monthly Next Memphis reports and quarterly Program Information reports.
- Keep Family Service /Health Supervisor and other Managers informed of programmatic issues in a timely and concise manner.
- Perform research/study to remain current of the trends involving early childhood education, the needs of children at risk and services to families affected by substance abuse.
- Contribute to team effort by performing other duties as assigned.
Physical Demands
While performing the duties of this job, the employee is regularly required to sit and talk or hear. The employee frequently is required to use hands to finger/handle/feel. Occasionally required to stand and walk. The employee must occasionally lift and/or move up to 20-30 pounds. Specific vision abilities required: close and color vision. Work in a non-smoking environment.
Special Conditions
Maintain a valid driver's license with a good driving record with appropriate level of insurance.
