What are the responsibilities and job description for the Disease Intervention Specialist I (LPN) position at Portsmouth City Health Department?
Position Summary:
Under the direction of the Portsmouth City Health Department’s HIV/STD Program Supervisor, the Disease Navigator provides HIV, syphilis, and Hepatitis C Virus (HCV) educational materials, testing, and linkage to care services in high-risk counties. Carries out job duties in a manner that supports the mission, vision and values of the Portsmouth City Health Department.
Essential Duties:
(70%) Conduct HIV screenings and risk assessments. Provide Syphilis, HIV, and HCV testing to high-risk clients with a finger stick test, risk reduction plan, posttest counseling and linkage to care services. Provide outreach services to high-risk areas in regions 6 & 7 that includes Syphilis, HIV, and HCV testing. Provide HIV screening at CTR sites, Syringe Exchange Programs, community events that will include occasional weekend hours.
(20%) Collaborate with DIS Supervisor/Nurse Educator to create resource guide for regions 6 & 7. Keep records of collaborations, resources, and services for each county. Provide educational materials to community partners on HIV, syphilis, and HCV resources, testing, and treatment.
(10%) Will work the Exchange program 1-2 times per month each Thursday 1pm– 6:30 pm.
Other Duties & Responsibilities
Stay current on HIV, syphilis, and HCV testing, treatment, and prevention guidelines. Perform other duties as assigned.
Minimum Qualifications:
At least one year of experience in a medical or public health setting. Associate degree in a health-related and/or Phlebotomy certification preferred. Valid Ohio driver’s license. Proficiency in Windows and Excel; ability to learn and utilize additional software programs as needed. Strong interpersonal and customer service skills.

Pay: $22.58 - $24.00 per hour
