- DEA with prescriptive authority required
- FNP designation with AZ license eligibility
- Experience in primary care preferred
- Provider who is comfortable with all ages
- Patient-focused, friendly and willing to collaborate in a team environment
- Flexible scheduling: 4/10-hour shifts offered
- Neighborhood clinic treating all ages
- Medical Assistants, Front Office, and leadership support
