Case Manager/Social Services

Job Summary
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate has a BA in Health Science, Social Work, Health Care Administration, BSN/RN or criminal Justice. As a Case Manager, you will play a crucial role in supporting clients by coordinating services, that promote their well-being and independence.

Duties
- Manage and oversee case management in the Illinois Department on Aging's, Community Care program and Adult Protective Service programs, ensuring compliance with relevant policies, documentations and standards.
- Develop individualized service plans tailored to the unique needs of clients, focusing on their needs and goals.
- Build and maintain positive relationships with clients, families, and community resources to facilitate access to necessary services.
- Monitor client progress and adjust service plans as needed, advocating for clients' rights and needs effectively.
- Conduct assessments to identify clients' needs, strengths, and challenges in order to inform program development and assure financial eligibility.
- Maintain accurate documentation of case notes, service plans, and client interactions in accordance with administrative requirements.

Requirements
- Bachelor’s degree in social work or a related field
- Excellent administrative skills with attention to detail for maintaining accurate records.
- Strong interpersonal skills to effectively communicate with clients, families, and team members.
- Ability to manage multiple cases simultaneously while maintaining a high level of organization and professionalism.
