POSITION DESCRIPTION
Reporting to the Clinical Director, the primary role of the RN Case Manager is to provide quality care and compassionate service consistently and according to the care plan, providing Clinical Nursing Care. As RN Case Manager, your role is also to provide supervision, training and support to clinical staff assigned.
RESPONSIBILITIES
Reporting to the Clinical Director, the RN Case Manager’s primary role is to provide quality care and compassionate service consistently and according to the care plan, providing Clinical Nursing Care.
· Serve as primary case manager for patients assigned
· Assist in assessing patient’s medical condition/status
· Assist with performing specialized procedures
· Prepare, secure, administer, and record patient medications
· Receive and renew medical orders
· Collaboration with physician and clinical director
· Provide accurate education to family and patient
· Follows each patient’s plan of care
· Document clinical progress
· Maintains confidentiality
· Serves on-call during rotation
· Supervises LPNs, TMAs, CNAs, and HHAs as assigned care team members
CARE TEAM:
· As case manager, play the lead role in coordinating for the patient’s care plan, collaborating and communicating with other members of the care team as needed to ensure that we are providing the highest quality of care and compassionate service.
PATIENT CARE
· Ensure that each patient receives quality clinical care and compassionate service consistently and according to the care plan.
· Clinical Skilled Nursing Care:
o Performing physical and psychosocial assessments on all hospice and home care patients as assigned.
o Assessing and reassessing pain.
Utilizing appropriate pain management techniques that are consistent with the individual patient’s care plan.
Educating the patient and family regarding pain management.
Assist in developing a pain management and symptom management plan to keep the patient as comfortable as possible. Reassessing pain and notifying the Clinical Director and/or Medical Director when pain is not manageable with the current medications prescribed.
o Working with Clinical Director and Care Team to develop a plan of care for each patient.
o Identifying, communicating and documenting patient’s status, changes in pain, symptoms, etc. and documenting the care or services you provide.
o Understanding and consistently complying with Quality Reporting Standards.
o Consistently and accurately utilize Electronic Medical Records system for reporting and recording information, including patient information, time sheet, etc.
o Providing skilled nursing care.
o Responding to the physical, mental, social and emotional needs of skilled and or terminally ill patients and their families. Offering ongoing support and palliative care.
o Assessing patient’s condition, initiating plan of care, re-evaluating and updating as necessary.
o Following up with obtaining lab results and notifying physician of any significant results.
o Performing patient care responsibilities considering needs specific to the standard of care for patient’s age and clinical condition.
o Demonstrating knowledge of medications and their correct administration based on the age of the patient and the patient’s clinical condition.
o Following the seven (7) medication rights and reducing the potential for medication errors.
o Performing all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health errors.
o Obtaining all necessary physician orders and collaborates with physician as patient’s needs dictate.
o Initiating needed referrals to other disciplines and/or community resources as needed to meet the patient’s needs.
o Supervising other Hospice care givers and volunteers as appropriate.
o Participate in by-weekly Interdisciplinary Team meetings to discuss multidisciplinary team responsibilities, patient’s condition/status, pain management, patient and/or family bereavement counseling needs, other needs, and recommendations, etc.
o Notifying Clinical Director of patients who need to be seen on weekends or holidays and submitting updated nursing care plan to the weekend/holiday nurse who will be providing care for the patient.
o Obtaining/ordering needed supplies and equipment to provide patient care.
o Addressing and supporting cultural practices, as long as such practices do not harm others, and regardless of whether or not they reflect your own beliefs.
o Adhering to all organization, payroll, billing and documentation policies and procedures consistently at all times.
o Participating in the organization’s other mandated activities, i.e. in-service meetings and trainings, staff meetings, etc.
· Provide a comfortable atmosphere and emotional support for patients and their families.
· Be present for patients and their families.
o Invite conversation, but respect the right of each patient and/or family to choose not to converse
Encourage conversation about the patient’s life experiences.
Invite questions and open dialogue about the patient’s care, expectations, wishes, etc.
o Respect the wishes and needs of the patient and/or family.
o Respect the privacy of each patient and their family.
o Listen – or just “be there.”
· Maintain a clean, safe and healthy environment for patients.
· Complete at least 12 in-service training hours annually in topics relevant to home and/or hospice care.
· Assist patients with transferring and ambulation as needed. May include: repositioning, walking, range of motion exercises, prescribed exercises, or etc. Moderate to heavy physical duty. Considerable standing, walking, bending, pushing, pulling, sitting, stooping, reaching, and lifting in excess of 35 pounds or more when assisting to move residents. Must be able to properly assist patients in transitions, repositioning and moving with proper lifting and transitioning techniques. Proper lifting and transitioning techniques include assistance, use of another staff when needed, use of lift when needed. Must also be able to use your abdominal and lower back muscles to support your body repeatedly or continuously over time without injury or fatigue.
· Perform all other duties as assigned.
SUPERVISION
· Supervise staff assigned to your supervision, working closely with each of them to build a cohesive team and a positive work environment where every employee feels valued and supported.
· Continually and consistently train, mentor, coach, encourage and support employees assigned to your supervision, recognizing the important role you play in their professional development and success as an employee and caregiver.
· Provide training for new employees assigned to your supervision and support them in their efforts to become acclimated in their new role.
· Ensure that employees under your supervision understand and follow agency policies and procedures.
· Ensure that employees under your supervision understand basic regulatory requirements that apply to their role in providing hospice, home care and/or assisted living services.
COMPLIANCE, DOCUMENTATION & REPORTING:
· Acquire a sound working understanding of regulatory/compliance policies and procedures.
· Follow all policies and procedures established by Prairie Home Hospice and Community Care and assist in ensuring that Prairie Home Hospice and Community Care meets or exceeds all state, federal, and industry standards for hospice or home care as it relates to compliance and regulations.
· Acquire a sound understanding of documentation policies and procedures for documenting patient care and services and ensure that you are following all documentation policies and procedures adopted by Prairie Home Hospice and/or required by state and/or federal regulations.

OTHER RESPONSIBILITIES & QUALIFICATIONS
· Must be able to serve on-call during rotation
· Effectively communicates patient needs to Clinical Director and/or Medical Director
· Communicates volunteer needs to Social Worker and/or Volunteer Coordinator
· Participates in efforts to create a positive work environment within Prairie Home Hospice and Community Care
· Participates in and supports Prairie Home Hospice and Community Care programs, events, fundraisers, etc., and makes every effort to contribute positively to the overall success of the organization.
· Provides input and feedback to management in efforts to provide quality care, compassionate service and a positive, healthy work environment across the organization
· Completes educational requirements necessary to maintain licensure.
· Maintains patient, staff, and organizational confidentiality at all times.
· Represents the organization in a positive and professional manner in the community.
· Arrives on time as scheduled for work shifts and/or patient visits.
· Must have the ability to work independently with good time management and organizational skills.
· Must have the ability to exhibit compassionate, dignified care to all patients at all times.
· Must be able to submit timely documentation of clinical analysis and patient visits.
· Attends staff meetings and trainings, as assigned.
