TASKS AND RESPONSIBILITIES
- Treat all individuals with dignity and respect while maintaining agency standards of confidentiality, professionalism and objectivity.
- Provide oversight of the full day and primary residential transition programs
- Provide therapeutic conditions of warmth, genuineness, empathy, respect, dignity and direction to all patients.
- Attend staff consultation, scheduled meetings, supervision and training activities.
- Facilitate transition groups with the men and women weekly.
- Coordinate transportation by insurance companies to get patients from place to place.
- Help patients identify and schedule aftercare plans while aiding in increase of patient personal autonomy.
- Assist in evaluating and selecting programs that produce results.
- Set up transition needs for patients who are asked to leave the program.
- Assist in evaluating and selecting programs that produce results.
- Coordinate discharge planning goals with the patients primary counselor.
- Create and maintain collaborative community relationships throughout the service area.
- Oversee MCO care coordination and communication with Medicaid providers as applicable with the full day and residential patient population.
- Administer PHQ-9 and appropriate referrals to Foundation II.
- Develop and maintain a community referral network. Demonstrate the ability to work collaboratively with community partners, exhibiting effective communication, cooperation, and facilitation skills to create a coordinated referral process for individuals as they transition.
- Maintain all regulatory, contract and documentation standard and data collection, and complete necessary documentation in a timely manner.
- Develop and maintain a working knowledge of all documentation and reporting systems to include but not limited to IBHRS, Accreditation Now, MyOutcomes, ASAM, and Electronic Health Record.
- Proficiency in clinical practices identified as best practice by the agency demonstrated through supervision and direct observation.
- Demonstrate understanding and adherence to all CARF and licensure policies and procedures.
- Perform all other duties as assigned by supervisor.
