Preceptor Home Health & Hospice is growing!

We are a home health and hospice agency providing services in patient homes in 16 counties throughout Wisconsin. Patient homes include private residences, assisted living facilities, memory care units, and resident care apartment complexes. Preceptor strives to provide high quality care through our multifaceted programs including home health, hospice, palliative care, and outpatient services. We have branch offices located in Germantown, Fond du Lac, Waukesha, and Appleton.

To best provide for our clients and case managers, we are looking to add to our 7 on/7 off team.

We have an opening for a Licensed Practical Nurse to cover our Southern Territories

Shifts are Monday through Thursday from 4 PM to 7:30 AM and from 4 PM Friday to 7:30 AM Monday on alternate weeks.
During this time, you will have both home health and hospice visits. Some of the visits will be scheduled visits and some will be on-call.

Job Requirements:

• Carry a current licensure in field of practice in the State of Wisconsin

• One year of Hospice experience required

• Excellent communication skills with patients, families, facility staff, doctors, and teammates

• Ability to work independently

• Good organization and effective time management skills

• Willingness to travel to meet patient needs

• Compliance with documentation standards and regulations

• Prior home health experience preferred, but does not affect eligibility to apply
