Certified Nurse Practitioner
Preferred Healthcare is a rapidly growing provider of specialized pulmonary consults in nursing homes and assisted living facilities. In addition to pulmonary care, we deliver compassionate, high-quality primary care for homebound seniors across the Cincinnati area.
We are seeking two dedicated and experienced Nurse Practitioners to join our team and provide in-person care in residential homes, assisted living communities, and long-term/post-acute care facilities.
Key Responsibilities:
- Deliver comprehensive primary care services to a geriatric population with complex, chronic conditions
- Conduct pulmonary consultations as appropriate
- Coordinate care with interdisciplinary teams across various settings
- Maintain accurate electronic documentation and adhere to best practices in clinical care
- Foster meaningful relationships with patients, caregivers, and facility staff
Qualifications:
- Active Nurse Practitioner license in the state of Ohio
- Minimum of 1 year of experience in geriatrics or chronic disease management
- Strong clinical decision-making and assessment skills
- Proficient in electronic health records and digital tools
- Excellent communication and interpersonal skills
