Job Overview
We are seeking a compassionate and dedicated Registered Nurse (RN) or LPN to join our Home Health team. The ideal candidate will provide high-quality nursing care to a single patient in their place of residence, ensuring their comfort and well-being. This role requires a strong background in various nursing practices, including infectious disease care, pain management, and catastrophic care, while adhering to HIPAA regulations.
Responsibilities
- Conduct comprehensive physical examinations and assessments of patients in their home environment.
- Develop and implement individualized care plans based on patient needs and medical history.
- Administer IV infusions and medications as prescribed by healthcare providers.
- Assistance with various in home PT/OT/ ROM Therapy
- Provide education to patients and families regarding health management, disease prevention, and treatment options.
- Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
- Collaborate with interdisciplinary teams to coordinate care for patient.
- Maintain accurate documentation of patient care activities in compliance with HIPAA standards.
- Offer support in pain management strategies tailored to individual patient needs.
Qualifications
- Current Registered Nurse (RN) license in the state of practice.
- Experience in home health care, nursing homes, or assisted living environments is preferred.
- Knowledge of anatomy and physiology relevant to patient assessments and interventions.
- Familiarity with infectious disease protocols and best practices in senior care.
- Strong skills in performing physical examinations and understanding ER protocols when necessary.
- Excellent communication skills to effectively interact with patients, families, and healthcare teams.
- Ability to work independently while managing multiple patient cases efficiently.
