Home Health RN / LPN

Job Overview
We are seeking a compassionate and dedicated Registered Nurse (RN) or LPN to join our Home Health team. The ideal candidate will provide high-quality nursing care to a single patient in their place of residence, ensuring their comfort and well-being. This role requires a strong background in various nursing practices, including infectious disease care, pain management, and catastrophic care, while adhering to HIPAA regulations.

Responsibilities

• Conduct comprehensive physical examinations and assessments of patients in their home environment.
• Develop and implement individualized care plans based on patient needs and medical history.
• Administer IV infusions and medications as prescribed by healthcare providers.
• Assistance with various in home PT/OT/ ROM Therapy
• Provide education to patients and families regarding health management, disease prevention, and treatment options.
• Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
• Collaborate with interdisciplinary teams to coordinate care for patient.
• Maintain accurate documentation of patient care activities in compliance with HIPAA standards.
• Offer support in pain management strategies tailored to individual patient needs.

Qualifications

• Current Registered Nurse (RN) license in the state of practice.
• Experience in home health care, nursing homes, or assisted living environments is preferred.
• Knowledge of anatomy and physiology relevant to patient assessments and interventions.
• Familiarity with infectious disease protocols and best practices in senior care.
• Strong skills in performing physical examinations and understanding ER protocols when necessary.
• Excellent communication skills to effectively interact with patients, families, and healthcare teams.
• Ability to work independently while managing multiple patient cases efficiently.

Job Types: Full-time, Permanent

Pay: $40.00 - $42.00 per hour

Expected hours: 36 per week
