Intake Specialist (NYC)

Premier Home Health Care Services, Inc. is a leading established Home Health Care Agency. We currently have an opportunity for an Intake Specialist in New York City.

The role of the Authorizations Specialist is to procure authorizations for services provided to the agency’s clients. Additionally, the Authorization Specialist will enter, scan and maintain the authorizations per established company protocols.

Qualifications:

• A minimum of two years of work experience.
• A minimum of one year of Home Care/Home Health Care experience.
• Strong attention to detail and follow-up skills.
• Ability to use computerized systems.
• Ability to balance multiple tasks and manage conflicting priorities.
• Bilingual English/Spanish required.

Essential Job Responsibilities:

• Accept intake of cases from all referral sources and log into appropriate office’s referral log. Compiles new patient information from referral source assuring accuracy in communicating to RN, maintaining accurate records.
• Responds professionally to all incoming calls within a designated time frame.
• Interact with clients and client families to ensure quality care and client satisfaction, through patient perception calls, filling out appropriate forms for each call.
• Initiates progress toward agency growth, maintaining relationships with referral sources. Assist in the preparation of office operational plan and participate in plan implementation.
• Assist team with paperwork/visits if time allows.
• Provide input and quality improvement suggestions regarding intake process to management.
• Verify smooth transition from office hours to on-call staff person through email communications.
• Advise the RN Supervisor of new client intakes and client changes, and provide appropriate paperwork, including intake, service agreement and payment option for RN who opens case.
• Schedule RN evaluations for Medicaid referrals and assign to RN in Pocket Home Care. Follow up with RN to assure completion of RN evaluation for Medicaid patients.
• Mailing out of brochures and introductory letters to prospective clients.
• PA authorization process and Tracking.
• Performs other duties as assigned.
