Key Responsibilities
- Engage in interdisciplinary discussions to review scheduled patients and their treatment plans.
- Conduct evaluations for new admissions to assess needs and create tailored treatment strategies.
- Collaborate on potential discharges and communicate effectively with facility staff regarding floor programs.
- Facilitate staff training sessions and provide updates on patient progress.
- Implement adaptive programs involving wheelchairs, splints, and other necessary equipment.
- Communicate patient progress, updated goals, and changes in status with families and facility team members.
- Strong clinical expertise in physical therapy practices.
- Exceptional communication and interpersonal skills.
- Ability to work effectively within an interdisciplinary team environment.
- Proficiency in evaluating and treating a diverse range of patients.
- Flexibility to adapt to the varying needs of patients and treatment settings.
- Valid Physical Therapy license in the state of New York.
- Experience in a skilled nursing or rehabilitation setting is preferred.
- Strong organizational and time management skills.
