Key Responsibilities
- Engage in interdisciplinary discussions and reviews of patients scheduled for treatment.
- Conduct evaluations for new admissions, ensuring personalized care plans.
- Communicate updates regarding potential discharges and coordinate with floor programs.
- Provide training to staff and share insights on patient progress.
- Develop and manage programming for wheelchairs, splints, and adaptive equipment.
- Liaise effectively with families and facility staff to discuss patient progress and set updated goals.
- Strong assessment and evaluation skills.
- Excellent interpersonal and communication abilities.
- Collaborative mindset, capable of thriving in a team-oriented setting.
- Proficient in creating individualized treatment plans.
- Experience with assistive devices and patient programming.
- Current Physical Therapist license in the state of Ohio.
- Relevant experience in physical therapy, particularly within skilled nursing settings is preferred.
