Requirements
- Graduate of a CSWE accredited bachelor’s degree required
- Master’s degree preferred
- One-year experience of supervised work in a health care setting required
- Capacity to effectively communicate with and assess residents
- Responsible for assuring residents rights are met and residents, family members and team members on resident’s rights as necessary.
- Meets with resident/family upon admission to assess and plan to meet psychosocial needs.
- Completes social history and assessment on each resident upon admission, if qualified.
- Completes appropriate documentation upon admission, transfer, discharge, and completes ongoing progress notes at intervals required by regulations.
- Maintains a current community resource file for education and communication of community resources, external referral sources to residents and families.
- Actively participates, and encourages resident/family participation, in the Interdisciplinary Care Planning Conferences.
