We need a proven clinicianwho willnot onlyhelp our team members and Network leaders as theysupport the daily living needs of our residents, but who alsodesires todeveloptrusting, caring relationships.TheLPN Case Manager isrequired to:
- Be a graduate of an approved School of Practical Nursing and have a minimum of 1 year of case management experience;
- Possess a valid LPN license issued by the State of Pennsylvania and maintain current CPR certification, per policy;
- Demonstrate strong interpersonal skills to support, advise, coach, and influence others while modeling enthusiasm and motivation;
- Be self-directed, set priorities, and have excellent time management skills;
- ExhibitproficiencywithEMR software as well asPC and Microsoft Office applications.
