Our client is seeking a PACE Scheduler to coordinate participant appointments and ensure smooth care delivery for older adults. In this role, you will be responsible for scheduling, documentation, and communication while working closely with interdisciplinary teams to enhance participant well-being.
Key Responsibilities
- Coordinate and schedule participant appointments.
- Facilitate external medical appointments as needed.
- Organize new participant orientation and initial assessments.
- Optimize scheduling to ensure efficient care delivery across multiple settings.
- Collaborate with key teams and departments to maintain smooth operations.
- Serve as a liaison between departments, ensuring seamless communication.
- Provide participant communication and follow-up to ensure engagement and care continuity.
- Manage scheduling conflicts and urgent requests efficiently.
- Maintain accurate documentation in electronic medical records (EMR) systems.
- High School Diploma or GED required.
- 2 years of experience in all of the following:
- Medical Scheduling
- Electronic Medical Records (EMR) systems
- 1 years of experience in at least one of the following:
- Frail Adult Care
- Geriatric Care
- Geriatric/LTC (Long-Term Care)
- Bilingual in Spanish (required).
- Must reside within 10 miles of Downtown Los Angeles.
- 1 years of experience in one or more of the following:
- Value-Based Care
- PACE (Program of All-Inclusive Care for the Elderly)
- Monday – Friday, 8:00 AM – 5:00 PM
- On-site position
