Job Title: Community Engagement Specialist
Job Summary:
We are looking for a dynamic Community Engagement Specialist to join our team. The ideal candidate will be responsible for fostering community engagement and promoting social well-being. As the Community Engagement Specialist, you will play a crucial role in connecting with young transgender and nonbinary individuals of color (YTGPOC), and linking them into services to support optimal health and wellness outcomes
Responsibilities:
1) Conduct HIV/STI/PrEP/PEP testing and screening services within an HIV status-neutral model of care. This includes conducting behavioral risk screenings, social determinants of health screening, providing personalized cognitive counseling, and linking clients into case management to promote ongoing care engagement regardless of HIV status.
2) Conduct outreach to identify YTGPOC in need of case management, HIV/STI testing, and/or PrEP/PEP support. This includes attending community events, social media postings, app-based outreach.
3) Conduct PrEP/PEP evaluations with high-risk individuals with a particular focus on YPTOC. This includes coordinating linkage into medical services with ongoing monitoring for all PrEP/PEP enrolled clients.
4) Provide ongoing PrEP/PEP support services which may include client escorting, insurance navigation, care coordination, biomedical screenings, appointment reminders, and adherence/retention counseling.
5) Carry a caseload of case management HIV Navigation Services (HNS) clients. This includes but is not limited to developing client-centered action plans, service/provider coordination, client follow ups, file organization, and data service tracking.
6) Assist with the identification of key YTPOC influencers to promote testing services within their social networks
7) Identify new medical and essential support service providers who have a history of providing services to YTPOC community. This includes attending initial meetings, assisting with the formalization of working agreements, and tracking agreement renewals.
8) Develop and execute new internal date-to-care program in collaboration with data team and PFY Director with the goal of re-engaging out-of-care persons living with HIV/AIDS into care.
9) Provide linkage support to HNS for all out-of-care person to promote engagement in essential and/or medical support services (ex: food pantry, housing assistance, insurance navigation, name change)
10) Promote the initiation of, retention in and adherence to, client centered sexual health based medical interventions.
11) Maintain all required electronic and written records of testing, community advisory board meetings/activities, linkages and outreach activities, in addition to maintaining monthly statistics of clients served.
12) Additional activities as assigned by supervisor.
Requirements:
Bachelor’s degree or some college in related field required. Professional or lived experience working with or being a trans person of color is desirable. Knowledge of sexual health prevention services and/or public health knowledge a plus. Candidate must be comfortable being outgoing and speaking to individuals from various backgrounds/experiences. Ideal candidate will be self-motivated, have strong organizational skills, exhibit a high level of trustworthiness, and have strong interpersonal skills. Excellent written and oral communication skills. Work schedule requires both late evening and weekend scheduling. Travel throughout Nassau, Queens and Suffolk Counties is required – must have own transportation (car and driver’s license). Bilingual English/Spanish is a strongly preferred.
