Position Overview:
Pride Health is seeking a skilled and experienced Social Worker with a valid LMSW license to join our team for a contract position specializing in discharge planning. This role is ideal for a professional with expertise in discharge planning and a commitment to providing high-quality care. The position involves assessing patients, creating discharge plans, and ensuring smooth transitions from hospital care to home or other care facilities.
Key Responsibilities:
- Discharge Planning: Work with healthcare providers to develop discharge plans, ensuring patients are discharged safely and efficiently.
- Patient Assessment: Evaluate patient needs, medical history, and family dynamics to ensure appropriate post-discharge care is arranged.
- Coordination of Care: Collaborate with interdisciplinary teams to implement discharge plans and ensure patients receive necessary follow-up services.
- Documentation: Maintain accurate, thorough documentation of discharge plans, patient assessments, and interventions.
- Patient Advocacy: Act as a patient advocate to ensure they understand their discharge instructions and have access to appropriate post-care resources.
Qualifications:
- Licensure: Must have a valid LMSW license issued by the New York State Education Department (NYSED), with primary source verification required.
- Experience: At least 1 year of experience in discharge planning.
- Skills: Strong communication, assessment, and discharge planning skills.
- Languages: Proficient in English (reading, writing, and speaking).
