The Care Manager will ensure all providers involved are up to date with case information and collaborate with patients' families, friends and care giver to effectively service the client. The Care Manager will continually develop a network of quality care givers and referral sources and keep accurate records. Very high level of organization and can complete the job requirements without constant supervision.
Responsibilities
- 10 Years working experience verifiable.
- 2 years work experience in health care or a related field.
- 5 years current management experience -large groups of people preferred-.
- Proficient with Healthcare Management Software and Microsoft Office Word and Excel.
- Networking knowledge and development of a business is necessary.
- Ability to travel locally.

- Creating and updating schedules for clients and care givers.
- Collaborating with the care giver and insurance case managers to ensure care is being delivered.
- Identifying and recruiting care giving staff.
- Ensuring client and care giver requirements are met.
- Conducting regular follow-ups with clients and care givers to evaluate progress, promote continuity of care and ensure improved outcomes.
- Maintaining records of case management activities and report to the insurance any changes.
- Complete EVV visits in the software daily.
- After hours and weekend on call is required.
- Complete home visits as required
- Network with the insurance case managers to get new referrals and grow the census of the office. This is a must have: if you are able to recruit staff and work closely with referral sources, this is the job for you. This is not just an office job it is a job that requires someone to grow the census and have GREAT case management skills.
