Position Summary
Clinical care teams, particularly those that integrate a clinical pharmacist, can provide enhanced support around medication use to drive better patient health outcomes and reduce the total cost of patient care. The incumbent will be responsible for supporting the work of the Clinical Pharmacist in building a comprehensive pharmacy support program, integrated into Primary Care IPA practices, in the pursuit of the Quadruple Aim.
Responsibilities and Duties:
· Under the direction of the Clinical Pharmacist, contact patients flagged for potential adherence concerns.
· Assist the Clinical Pharmacist in the identification of high-cost medication usage.
· As directed by the Clinical Pharmacist, contact patients to carry out medication related care plans.
· Serve as a resource to physicians, nurses, and other team members on pharmacy matters relating to patient care.
· Identify and facilitate cost-saving opportunities for patients
· Work with health plans on authorizing physician-prescribed medications.
· Manage the appointment schedule of the clinical pharmacist
· Answer and triage incoming calls to the clinical pharmacy department
· Document all patient encounters into the electronic medical record (EMR)
· Accept and perform other duties as assigned.
Qualifications and Requirements
· Certified Pharmacy Technician credential (CPhT), preferred (not required)
· 3-5 years’ experience in community pharmacy, preferred (not required)
· Good written and verbal communication
· Attentive to detail
· Empathetic to patient needs
· Outstanding customer service skills
· Ability to multi-task and keep organized
· Ability to work cohesively within a team
· Computer skills including Microsoft Word, Excel, PowerPoint, and Outlook
