Triage Nurse / Triage Medical Assistant
Job Summary:
The Triage Nurse or Triage Medical Assistant is the advocate between the provider and the patient. This position gives our patients supervision, direction, and coordination of nursing care.
Duties/Responsibilities:
Assure positive communications among fellow nurses, physicians, patients, and families
Work independently utilizing critical nursing skills
Maintains positive interface and relation with outside organizations such as other physician offices, other health care providers, etc.
Communicates clearly with patients relaying information from providers and other sources. Assess the patients needs and relays to the provider any concerns.
Responds to emergent health conditions for patients in office and at Diagnostics
Management of and Prioritize severity of Triage tasks and incoming calls including critical such as: Chest pains, Shortness of breath, abdominal pain, injuries (both critical and simple)
Reporting of diagnostic testing, referrals, ordering & scheduling of tests
Home Health, Hospice referral, and Nursing Home placements and the coordination of care with outside facilities
Refilling and monitoring the use of medications and pain management, implementing the chronic Opioid protocol and the calculations of the MEDD. Communicates with patients regarding prescription need, clarification and collaborating to be sure patients have correct understanding.
Responds to all emergent phone calls and EMR messages to assess and give directives
IV therapy if certified
Case Management of the following protocols and implementing the action plan and prescribing of medications; UTIs, Strep, Vaginal Candidiasis, COPD, CHF, Asthma, and Coumadin, chronic opioids and pain management.
Supporting floor staff when needed (blood draws, catheters, injections)
Authorization for special services (DME, Infusions, in office injections) and appeal denied services
Answer/Research insurance issues for Physicians and Staff
Completion of forms, (FMLA, ULTC, school forms and physicals, CMN, insurance and workers compensation forms, diabetes forms)
Orders DME and supplies facility with necessary information. Follows up on DME completion.
Updates DME guidelines that Providers need to use for DME approval.
Experience:
Must be an RN or LPN, or Medical Assistant who is capable of working in a the triage position; caring personality with a positive attitude; calm under pressure and exhibit sound nursing judgment; excellent time management and problem solving abilities, good communication skills. Must have experience in pharmacology. IV certified helpful. Good computer skills, detail oriented and self- motivated.
Education Requirements:
Have appropriate education and current license for an LPN or RN or Medical Assistant
Working Conditions:
Works in a well-lighted area; primarily seated at a desk, extensive telephone and computer work
