Key Responsibilities :
Maintain accurate and organized patient records in eClinicalWorks (eCW) by scanning, updating, and tracking medical / dental documents.
Process and respond to medical records requests from patients, providers, attorneys, insurance companies, and government agencies.
Support care coordination by notifying Patient Navigators of incoming referrals and updating patient status alerts for key screenings (mammograms, PAP smears, colonoscopies).
Assist with prescription prior authorizations by forwarding requests to the appropriate providers.
Monitor and manage after-hours messages, ensuring prompt follow-up for patient care needs.
Facilitate communication regarding emergency room visits and hospital admissions to support follow-up care.
Maintain compliance with HIPAA regulations , OSHA standards , and Patient-Centered Medical Home (PCMH) principles .
Handle incoming mail, distribute to appropriate team members, and support administrative functions.
Uphold PHCCs values by treating patients, staff, and providers with dignity and respect.
Qualifications & Skills :
Education : High school diploma or equivalent required.
Attention to Detail : Strong organizational skills to ensure accuracy in records management.
Tech Savvy : Comfortable using electronic medical record (EMR) systems and office technology.
Communication : Professional and clear verbal / written communication skills.
Commitment to Mission : A passion for serving our community with empathy and respect.
Work Environment :
This position may have remote work requirements based on organizational needs.
Must comply with all CMS vaccine mandates as applicable.
