Key Responsibilities:
- Maintain accurate and organized patient records in eClinicalWorks (eCW) by scanning, updating, and tracking medical/dental documents.
- Process and respond to medical records requests from patients, providers, attorneys, insurance companies, and government agencies.
- Support care coordination by notifying Patient Navigators of incoming referrals and updating patient status alerts for key screenings (mammograms, PAP smears, colonoscopies).
- Assist with prescription prior authorizations by forwarding requests to the appropriate providers.
- Monitor and manage after-hours messages, ensuring prompt follow-up for patient care needs.
- Facilitate communication regarding emergency room visits and hospital admissions to support follow-up care.
- Maintain compliance with HIPAA regulations, OSHA standards, and Patient-Centered Medical Home (PCMH) principles.
- Handle incoming mail, distribute to appropriate team members, and support administrative functions.
- Uphold PHCC’s values by treating patients, staff, and providers with dignity and respect.
Qualifications & Skills:
- Education: High school diploma or equivalent required.
- Attention to Detail: Strong organizational skills to ensure accuracy in records management.
- Tech Savvy: Comfortable using electronic medical record (EMR) systems and office technology.
- Communication: Professional and clear verbal/written communication skills.
- Commitment to Mission: A passion for serving our community with empathy and respect.
Work Environment:
- This position may have remote work requirements based on organizational needs.
