Job Summary:
We are seeking a dedicated Registered Nurse Case Manager to join our team. The ideal candidate will be responsible for overseeing the coordination of patient care, ensuring efficient and effective delivery of services. The role involves collaborating with healthcare professionals to develop and implement comprehensive care plans for patients.
Duties:
- Utilize expertise in behavioral health, PICU, inpatient, and primary care settings to assess patient needs and provide appropriate interventions.
- Coordinate care for patients in an acute care setting, particularly in a Level I Trauma Center.
- Conduct thorough discharge planning to facilitate smooth transitions for patients.
- Engage in case management activities to optimize patient outcomes and promote continuity of care.
- Provide exceptional patient service by advocating for individualized care plans and addressing patient concerns promptly.
Experience:
- Previous experience in case management or related roles is preferred.
- Proficiency in acute care settings and familiarity with primary care practices.
- Strong background in behavioral health interventions and discharge planning.
- Excellent communication skills to interact effectively with patients, families, and multidisciplinary teams.
- Ability to work collaboratively in a fast-paced environment while maintaining a high level of accuracy and attention to detail.
