Job description :
Deliver general nursing care and related services to assigned patients in a home health setting.
Perform treatments using aseptic techniques including sterile dressings irrigations and cauterizations.
Assist in developing and implementing patient care plans based on individualized needs.
Provide functional direction to ancillary personnel to ensure efficient patient care delivery.
Maintain accurate and timely documentation in Homecare Home Base (HCHB) and OASIS.
Monitor patient conditions report any changes in health status and collaborate with the interdisciplinary team.
