The Admissions Nurse is an integral member of the interdisciplinary hospice team, specializing in supporting patients and families in the decision to begin hospice care. Admission Nurses are the first contact with patients and families, providing education and guidance on the hospice benefit while using strong clinical skills to evaluate eligibility per the Medicare guidelines. Hours are Tues. - Fri. - 12:00 pm - 10:30 pm
Position requirements:
The Admission Nurse is responsible for conducting evaluation and information visits to potential patients and their families, obtaining consents, election forms, performing an initial comprehensive physical assessment and developing a plan of care.
Must be a Registered Nurse
Sample Duties and Responsibilities:
Admission Process:
- Evaluates patients for hospice appropriateness using medical records review and consultation with Supervisor, DPCS and Medical Director.
- Explain benefits of hospice program and obtain signature of informed consent.
- Admit patients, develop plan of care and complete and submit relevant paperwork
- Obtain data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporate into plan of care.
- Utilize work time and resources effectively and efficiently by completing assigned tasks within specified time limits while maintaining high productivity and performing efficiently regardless of whether the work volume is high or low.
- Orders DME, Medications and supplies as needed.
- Build referral source relationships to support the growth of the hospice
- Maintain clinical and professional competence.
- Assist with the continuity of care and discharge planning from facilities.
- Following admission, ensure smooth transition of new patients to case manager assigned.
REQUIREMENTS:
- Current California RN license.
- 1 year of RN experience, 2 to 3 years hospice or home health experience preferred.
- Knowledge of state and federal regulations relating to hospice participation.
- Able to multi-task effectively and work as a team.
- Works well independently.
- BLS required within 60 days of hire. Excellent observation, diagnostic, medication management and clinical documentation skills.
- Travel required. Current California driver's license and proof of insurance.
