JOB TASKS AND RESPONSIBILITIES
- Provide administrative support to Practice Care Director
- Supervise front desk reception staff, including scheduling and training
- Oversee the Patient Engagement Communication Process
- Optimize and maintain providers’ schedule and coordinate provider follow-up appointments
- Execute patient outreach calls to ensure patient preventative services are achieved, i.e. Annual Physicals, Medicare Wellness Visits, Visit Programs, etc.
- Collaborate with partner payers to achieve mutually defined quality, health, and costs of care goals with Value -Base Programs (Horizon, AmeriHealth, MIPS, etc.)
- Assist in developing patient education materials

WHAT YOU SHOULD HAVE:
- Must be able to lead a team and work effectively and efficiently as part of the team
- Strong knowledge of medical terminology
- Very good interpersonal / human relations / people management skills
- Must be very comfortable and confident using technology and familiarity with EMR systems
- Knowledge of HIPAA, Meaningful Use or MACRA is an asset
- Must be well organized, detail oriented and able to communicate effectively
- Ability to maintain confidentiality
- Ability to respond to issues and questions in a tactful and professional manner
- Ability to establish departmental service standards
- Must have a strong work ethic.
Education:
- College Degree – Public Health / Health Care Management preferred, or Biology, Health Sciences or Health Care Informatics.
