You are a SLP who:
- Holds a current License/certification in Wisconsin
- Values building connections with their patients.
- Likes working independently while backed by a collaborative team.
- Has great written and oral communication
Job Duties Include:
- Provide Speech Therapy treatments to patients according to treatment plans
- Collaborate with the Interdisciplinary Team to identify and assist with obtaining patient specific DME including mobility aides, wheelchairs, cushions, orthotics, and adaptive equipment.
- Maintain daily records of treatments and document progress notes on patient.
- Collaborate with clinical team to develop and educate patient and/or care givers on safe discharge plan.
