Job Summary
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being while promoting independence. Duties
- Conduct comprehensive assessments of patients' health status, including diagnostic evaluations and medical history review.
- Develop and implement individualized care plans in collaboration with interdisciplinary teams, focusing on primary care and discharge planning.
- Monitor patient progress and adjust care plans as necessary, ensuring continuity of care.
- Provide education and support to patients and their families regarding health management, medication administration, and treatment options.
- Manage medical records accurately, documenting all patient interactions and updates in compliance with regulatory standards.
- Coordinate outpatient services and case management to facilitate seamless transitions of care.
- Engage in utilization management activities to optimize patient outcomes while ensuring efficient resource use.
- Address behavioral health needs as part of holistic patient care when applicable.
- Collaborate with Level II Trauma Center staff for patients requiring specialized interventions or follow-up.
Requirements
- Current Registered Nurse (RN) license in the state of practice.
- Previous experience in home health nursing or a related field preferred.
- Strong knowledge of ICU protocols and practices is a plus.
- Proficiency in diagnostic evaluation techniques and case management principles.
- Excellent communication skills, both verbal and written, with the ability to build rapport with patients and families.
- Ability to work independently while being part of a collaborative healthcare team.
- Strong organizational skills with attention to detail for accurate medical recordkeeping.
