Professional Case Management Services of America (PCSA) currently has openings in Dekalb, GA and surrounding counties. Responsibilities for this position includes:
1. Provide ancillary supervision and support to newly hired Support Coordinators as assigned.
2. Ensure that newly hired support coordinators (as assigned) facilitate all activities related to gathering information to be included in the support plan.
3. Ensure that newly hired support coordinators conduct monthly site visits and record visits accurately on the site monitoring form.
4. Provide support to active Support Coordinators.
5. Provide support to Regional Management team. Maintain a caseload.
Trainers maybe assigned a smaller caseload and will perform the following activities for individuals assigned to them:
1. Assist the individual and family in developing annual budget and implanting the ISP.
2. Oversee the person’s participation in the Services, supports and activities delineated within the ISP.
3. Complete all documentation which includes but is not limited to: CIRs, RCRS, DMA7, discharges, and transfers based on individual support needs.
4. Fulfills in accordance with the job description and all other duties as assigned by the upper Management.
Job Type: Full-time
Pay: $42,000.00 - $45,000.00 per year
