Position: Patient Service Representative

Department: Call Center

Reports to: Call Center Supervisor

Position Description:

tango is looking to hire a representative for our Start of Care Team. This position entails communicating with our patients who will be or are receiving home health services. Candidate is responsible for documenting and maintaining accurate records during the process starting with the initial contact made with the patient, delivery of home health care services and the final record of completion in our database.

Office Location:

Office located at 7600 North 16th Street, Suite 140 Phoenix, AZ 85020 (Remote)

Responsibilities and Duties:

Responsibilities of the Patient Service Representative include, but are not limited to the following:

• Making outbound calls to our patients about their home health services.
• Ensuring customer satisfaction and assisting them with issues/concerns related to their health.
• Contacting home health agencies to confirm services for our patients.
• Evaluates and determines caller needs and use their knowledge to provide resolution, tracking, and if necessary follow up from another representative
• Interdepartmental communication about home health referrals.
• Perform data entry in multiple systems for our patients.
• Other duties as assigned.

Qualifications:

• Must be at least 18 years of age.
• High School Diploma or GED, some college
• Ability to convey a positive and professional image to customers and employees
• Is customer service oriented with good oral and written communication skills.
• Capable of following an issue through to its conclusion
• Compassion and empathy a must.
• Strong attention to detail and critical thinking skills.
• Must be able to multitask.
• Proficient in Microsoft Office products.
• Is self-directed, flexible, cooperative, and exhibits the ability to work with minimal supervision.
• Must be flexible and willing to work weekends and Holidays.
• Home care/healthcare background is a plus but not required.
• Experience with speaking on the phone in a previous role preferred.
• Must have a secure work location from home with High-speed internet.
