Professional Home Care Services, Inc. is a mid-sized licensed home health agency. We strive to provide excellent care to our clients while fostering a rewarding family atmosphere for our employees. We are an established home health agency that has been in business for over 30 years.
We are recruiting for an experienced and highly dependable part-time Personal Care Worker (PCW) to work in Milwaukee, WI (N. 19th St & Capitol Dr). You will be assigned to one main client and a few part-time clients.
The schedule is: M-F during the AM shift and every other weekend (the weekend shift will be 4hrs max per day.)
This position requires at least 2 years of Caregiver experience, a driver’s license, and a dependable vehicle.
MINIMUM REQUIREMENTS:
- Personal Care Worker (PCW) with at least 2 years experience
- Previous home health care experience is required
- Experience caring for clients with MS, quadriplegia, dementia, etc. is a plus
- Solid Caregiver skills including transfers, catheter care, bowel program, range of motion, etc…
- PHCS is a Federally and State Licensed agency. This means that we provide skilled services to our clients. Our Caregivers are expected to provide a higher level of care. If you are looking for a sitter position, do NOT apply.
- Must have no physical restrictions and be able to pass a pre-employment physical and drug screen
- Must have a driver’s license and reliable insured vehicle to be able and willing to travel between client homes.
- Must have a positive attitude, be willing to work as a team with other caregivers, and accept and take direction.
- Must be able to keep a schedule, be on time, and not cancel assignments.
DESCRIPTION:
The PCW, under the supervision of the RN Case Manager, provides hands-on personal care and assistance with daily living activities to home care recipients, as directed by the RN Case Manager based on a written Caregiver Care Plan. The Caregiver reports observations and notes changes in patient conditions to the RN Case Manager immediately. The Caregiver documents appropriately and accurately the cares and observations from the home visit. The Caregiver uses Electronic Visit Verification (EVV) as a time and location log as required by Federal Law.
PRINCIPLE DUTIES:
- Provides non-skilled intervention and direct personal care, as directed by the RN Case Manager in the form of written instructions (Caregiver Care Plan). Such cares include: bathing, personal hygiene and grooming, toileting, skin care, oral care, hair care, nail care, foot soak, back rub, and dressing.
- Assists patient with transfer and ambulation activity, using proper body mechanics and maintaining proper alignment of the patient’s body. This may include lift transfers.
- Plans and prepares nutritious meals and snacks for patient, following therapeutic diet ordered on the Caregiver Care Plan. Encourages or restricts fluids as directed by the Care Plan. Shops for groceries, assists the patient with meal set-up, or feeding as directed by the Care Plan.
- Maintains a clean and healthy environment by performing the light housekeeping tasks and other household services to maintain patient independence at home, as directed on the Caregiver Care Plan.
- Acknowledges and complies with the Agency Exposure Control Plan, including wearing Personal Protective Equipment as appropriate.
- Performs or assists the patient in performing the following special skills as trained and directed by the RN Case Manager:
- Prescribed range of motion or other exercises.
- application of elastic leggings and/or sleeves
- transfers, use of transfer aids, special assistive appliances or equipment (brace, medical lifts including slide board, sling, splint).
- application of hot or cold compress and non-sterile dressings to intact skin
- performs catheter care, including changing the drainage bag or applying leg bag, and cleaning of all equipment.
- Participates in the home health agency’s In-Service Educational Training programs.
- Used Electronic Visit Verification (EVV) to turn in all documentation on time. This includes medical documentation (visit notes) and time logs.
- Willing to help cover cares for clients that are not part of your normal schedule when needed.
- Maintain a professional relationship with all clients no matter the situation (this includes clients that have psychological issues.)
