Physical Therapist – Home Care, Part-Time
Professional Physical Therapy is looking for a Licensed Physical Therapist to work part time as a Home Care PT.
Position Description: As a Home Care Physical Therapist, you will provide one on one therapeutic treatment and care to patients in their OWN home, to promote their health and independence using a comprehensive and patient specific care plan. Home Care patients have recently undergone total joint replacement procedures. The Home Care PT, when necessary, prepares patients for transfer to an outpatient physical therapy setting.
Essential Functions:
· Plans and administers medically prescribed physical therapy treatment for patients who are post operative total joint procedures.
· Reviews and evaluates patients’ conditions and medical records.
· Provides assessment of evaluation.
· Develops short and long-term treatment plans and measures patient progress.
· Plans and prepares treatment programs based on evaluation of patient data.
· Evaluates effects of treatment at various stages and adjusts treatment to achieve maximum benefit.
· Records treatment, response and progress in the patient’s chart and records it in the Electronic Health Record.
· Writes and updates progress notes; maintains all patient chart notes.
· Educates patients on home exercise programs.
· Provides discharge planning for patient including transfer to outpatient physical therapy facility.
· Acts as a liaison between patients and doctors.
· Supervises all activities in directing patient care.
· Adheres to all HIPAA and OSHA laws and regulations as required; obeys all applicable federal, state and local laws and regulations.
· Adheres to company policies.

- Must have a valid NJ Physical Therapist License.
- Must have a valid driver’s license and auto insurance.
- Doctorate or Master’s of Physical Therapy required.
- Must possess excellent interpersonal skills and the ability to maintain positive collaborative relationships across all levels.
- Must be able to communicate well with patients, co-workers and the general public.
- Must be punctual and dependable.
- Must have strong clinical skills.
- Conduct patient evaluations and develop individualized treatment plans.
- Provide hands-on therapy and instruction to patients.
- Maintain accurate patient records and progress notes.
