We are currently seeking a fully-licensed Mental Health Professional to join our Jefferson CCS Service Facilitation program. Our program provides case management services to individuals and families with severe mental health and/or substance use disorders, and are at-risk of psychiatric inpatient hospitalizations or other out-of-home placements. As the Mental Health Professional, you will provide clinical supervision and consultation to Service Facilitators and Direct Service Providers, to ensure our clients receive the highest quality of care.

KEY RESPONSIBILITIES OF THE MENTAL HEALTH PROFESSIONAL:
- Provide day-to-day consultation and regular clinical supervision to Service Facilitators to ensure effective and quality clinical programming for clients.
- Review cases, assess performance, and provide feedback to Service Facilitators; and supervise and guide Facilitators in appropriate service delivery.
- Assist with incoming client referrals.
- Monitor quality assurance through regular file reviews.
- Maintain data and generate reports necessary for ongoing tracking and reporting of participants.
- Act as a resource for workers in emergency and crisis situations.
- Authorize psychosocial rehabilitative services through CCS (Comprehensive Community Services) as the Mental Health Professional.
- Ensure the collaboration of service providers.
- Provide coverage as a Service Facilitator as needed.
SALARY: $70,000-72,000
LOCATION: Office is located in Fort Atkinson. Flexibility to work hybrid/remote 1-2 days per week.
SCHEDULE: Hours are typically Monday-Friday between 8:00am-5:00pm.

REQUIREMENTS:
- Must have a Master's Degree in Social Work, Psychology, Criminal Justice, or related.
- Must be a licensed mental health professional in the State of Wisconsin (LCSW, LPC, LMFT)
- Two years of previous experience working with youth and families is required.
- Must have reliable transportation on a daily basis, a valid driver's license and automobile insurance.
Salary : $70,000 - $72,000
