Job Summary:

The Qualified Intellectual Disabilities Professional (QIDP) is responsible for participating as a member of the facility team to ensure safe and effective residential services. The QIDP oversees all aspects of client care and assists with training support personnel. The QIDP is a positive role model, adaptable, mission oriented, goal driven, highly organized, efficient, and exercises good judgment and decision making skills.

ROTATING WEEKENDS AND ON CALL HOURS ARE REQUIRED FOR THIS POSITION

Job Duties and Responsibilities:

The statements below describe the general nature and level of work being performed in this role. They are not intended to be a complete list of all duties and additional responsibilities may be delegated as required:

1) Ensure that CILAs are in compliance with current policies and procedures, state and federal rules and regulations, and assist with ongoing development, reviews and revisions.

2) Provide feedback and support to direct care staff in conjunction with their supervisor, while maintaining a positive working relationship. This includes but is not limited to new employee orientation.

3) Attend Person Centered Plan (PCP) meetings that are organized by the ISC.

4) Develop and implement the implementation strategy plan that coincides with the PCP.

5) Ensure appropriate implementation of outcomes for the residents on your caseload.

6) Follow up with Implementation Strategy Plans (ISP) and ensure that staff is properly executing the outcomes assigned.

7) Review individuals’ needs with the community support team to ensure the individuals on your caseload are receiving appropriate services (annual and six month reviews).

8) Thoroughly complete quarterly environmental reviews and reports.

9) Complete monthly census for each client of caseload.

10) Complete monthly progress notes, monitor clients, and review all required documentation to ensure continuity of all clients’ care and programming needs.

11) Ensure that all files and reports are filed promptly in the client’s record.

12) Complete onsite day training visits at least bi-weekly and more often if necessary or as directed.

13) Monitor clients in their residential setting, at least weekly, to ensure appropriate implementation of programs and outcomes.

14) Review all client records on a quarterly basis to ensure that required documentation is completed timely, thoroughly, accurately, and if any changes are necessary

15) Attend monthly staff meetings, weekly management meetings, agency in-services, and other meetings as directed by the supervisor.

16) Schedule and chair all intake and discharge staffing, special team meetings and any other periodic reviews for client on your caseload.

17) Handle all complaints from individuals, guardians, agencies and families. Direct to the supervisor if unable to come to a resolution.

18) Monitor the electronic communication platform and ensure documentation is being completed.

19) Ensure that all clients are offered opportunities for community integration and that all clients’ rights are upheld and treated with dignity and respect.

20) Complete projects and special assignments by establishing objectives, determining priorities; managing time; gaining cooperation of others; monitoring progress; problem-solving; making adjustments to plans.

21) Ensure that all resident rights are upheld and all clients are treated with dignity and respect.

22) Complete annual redeterminations for assigned clients to ensure continuance of state benefits.

23) Required to go to the hospital to assist with the hand off of services and to ensure the rights of the individual are met.

24) Be on-call for any incidents, injury, or behavioral concerns.

25) Provide detailed written summary/follow ups to the IDT regarding all medical/behavioral incidents and/or hospitalizations.

Qualifications:

Education and Licensure:

1. Must meet State educational requirements for QIDP (Bachelor’s degree in a human service related field)

2. Must become an approved QIDP and DSP trainer with the State of Illinois

3. QIDP Certification Required

Other Requirements:

1) Must have valid driver’s license and comply with company policy for driving vehicles

2) Be willing to accept and respect supervision and assume responsibility for assigned work, follow established policies and procedures and follow instructions

3) Ability to handle sensitive information and situations discretely and confidentially

4) Excellent computer skills; Microsoft Office, Excel, PowerPoint, etc.

5) Attention to detail and accuracy of information

6) Project and goal orientation with strong organizational and interpersonal skills

7) Excellent written and verbal communication skills

8) Ability to prioritize and manage multiple assignments in a fast-paced environment to meet deadlines with efficiency and accuracy.

9) Must have a minimum of one year working with the DD population

Effort Extended:

Physical Demands:

1. Must be in good health and free of communicable disease.

2. Moderate to heavy physical exertion required to perform job duties. Duties will require reaching above and below the waist, bending, stooping, lifting up to 50 pounds, stretching, walking and standing, which may occur on smooth, rough, or uneven surfaces.

Mental, Visual and Aural Demands:

1. Must have the mental ability to understand and perform the requirements of the duties.

2. Must have functional hearing and vision.

3. Must be able to read, write and understand written English communication.

4. Constant mental, visual and aural attention necessary to perform duties satisfactorily.

5. Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, agitated, confused and sometimes hostile clients within the facility.

Public Interaction:

1. Must be tactful, courteous, diplomatic and exercise good judgment in all contacts with the public, by telephone or personally.

Pay: From $52,000.00 per year
