Job Title: Qualified Intellectual Disabilities Professional (QIDP- CILA)
Location: Schaumburg, IL

Our QIDP provides residential related case management services to persons served while working within an interdisciplinary team. This is an hourly position with a competitive wage of $25.13 per hour.

Essential Duties and Responsibilities:
- Responsible for ensuring the welfare, health, and safety of persons served along with Interdisciplinary team members.
- Responsible for writing and ensuring the ISP process occurs annually and six-month reviews are completed. Ensures monthly and quarterly review of ISP.
- Establishes and trains methodologies.
- Provides a written record of team meetings within 30 days of each meeting.
- Maintains communication with families/guardians and other residential staff and disciplines regarding overall status.
- Prepares and maintains written case records, reports, and forms, performs case follow-up and closing, and performs other administrative tasks, as required.
- Counsels persons served, assisting them to identify and resolve problems and make effective use of resources.
- Develops and maintains contact with other agency staff and local, day, and other service providers to develop specialized services for persons served.
- Develops and trains staff on Behavioral Management, Personal Safety Plans, Risk Management plans, or other specialized intervention plans.
- Advocates for the rights of the persons served.
- Identifies and addresses gaps in the service provision.
- Initiates and coordinates IDT meetings, as necessary.
- Ensures services are provided in accordance with CARF standards, funding sources and licensing agency requirements, and organizational policy and procedure.
- Prepares and compiles data, as requested.
- Trains new staff on case records, the specifics of the ISP, and agency and governmental regulations, as appropriate.
- Assists in development and communication of policy and procedures to ensure all regulations of the agency are adhered to.
- Supports the strategic planning process and may assist in developing the initiatives to achieve the overall mission and vision of the organization.
- Participates in organizational committees and meetings, as appropriate.
- Adheres to St. Coletta organizational or departmental policies and procedures.
Education and/or Experience: CILA QIDP: Bachelor's degree in related field which meets the requirements of 59 IL Administrative Code 115.120 and at least one-year experience of working directly with individuals with Intellectual/Developmental disabilities.
Computer Skills: Knowledge of Microsoft Office Suite Products, Internet, and email.
Certificates, Licenses, Registrations: Valid driver's license with a past driving record that meets the standards set forth by the organization.
Other Skills and Abilities: Ability to respond to behavioral and other emergency issues. Efficient time management and organizational skills. Ability to maintain confidentiality. Effective oral and written communication skills. Ability to establish and maintain effective professional working relationships. Skill with adults who have developmental disabilities.
Other Qualifications: At least one year of case management experience in a human service related field preferred. Knowledge of developmental disabilities. Knowledge of proactive case management practices that enhance the lives of the persons served. Ability to maintain a schedule that allows for visibility in assigned areas when persons served and staff are present.
