What are the responsibilities and job description for the Day Program Coordinator position at Progressive Healthcare Providers - Fern Park LLC?
Looking for an outgoing, organized leader to assist in running our Day Program for individuals with intellectual and developmental disabilities. This position will serve as the leader of the Day Program Instructors and will coordinate the day-to-day activities of the program.
Ensure the implementation of person centered supports in the habilitation process including personal care, daily activities and
community-based activities focusing on the participants choices. Ensures that quality of care, services and training are
provided to individuals in accordance with Federal, State, and local standards.
- Must be able to pass a criminal background screening and drug test.

- Must be able to read and write American English proficiently.

- Ability to understand, implement and monitor various software systems for training, record keeping and employee
- Must possess good time management and organizational skills to prioritize work for timely completion in a fast-paced
- Possession of a valid Driver's License with required endorsement, along with an acceptable driving record for the past three
- Ability to understand, apply and demonstrate the process of data collection.
- Ensures that communication is maintained with individuals, parents, concerned family and guardians as well all other service providers regarding all aspects of the individual.
- Participates in the admissions and discharge processes for individuals.
- Ensures staff performance is reviewed identifying both strengths and areas of concerns using multiple tools including reviews and appropriate disciplinary action in accordance with policy.
- Actively monitors programs and documentation of programs for service delivery and quality.
- Analyzes data and makes appropriate program interventions as needed to assist individuals progress.
- Completes Monthly, Quarterly and other assigned progress reports for all programs.
- Conducts all internal monitoring, completes monitoring reports and action plans based on findings.
- Completes corrective action assignments for internal/external monitoring in a timely manner.
- Provides leadership and supervision to assigned staff and assists with duties to ensure overall program consistency and compliance with applicable State and Federal standards, regulations, and guidelines.
- Bachelor’s Degree in Human Services field preferred.
- High School Diploma or GED required.
- Minimum of three (3) years of previous supervisory experience.
- Strong MS Office Suite Knowledge.

- Ability to work in close physical contact for providing personal hygiene assistance to individuals.
- Must be physically capable of transferring and lifting persons with or without reasonable accommodation.
