What are the responsibilities and job description for the QIDP/Program Specialist position at Progressive Healthcare Providers - Fern Park LLC?
The QIDP at Fern Park Developmental Center will coordinate the habilitation process for assigned individuals and assure that each individual’s quality of care, services, and training is provided in accordance with Federal and State Standards. Must have excellent writing and communication skills. Salary commensurate with experience.

- Ensures individual assessments are completed by Professionals/Consultants to determine the correct level of functioning within thirty (30) days of admission, at least annually and as needed.
- Facilitates the exchange of information amongst the interdisciplinary team to review and revise individual programs, schedules, strategies, and progress as needed.
- Ensures completion of all documentation regarding programming, health and safety, comprehensive assessments, including daily observation, log sheets, and daily reports.
- Ensure results of the Interdisciplinary Team decisions are implemented in a timely and appropriate fashion.
- Ensures opportunities for participation and active treatment is provided at natural times in multiple settings throughout the day to promote independence, reinforce skills and master behaviors.
- Ensures staff performance is reviewed identifying both strengths and areas of concerns using multiple tools including reviews and appropriate disciplinary action in accordance with policy.
- Train staff on multiple in-services, program implementation and other subjects using demonstrations, practice, observations and feedback methods to ensure staff mastery of technique is achieved.
- Ensures the development of Behavioral Intervention plans for all individuals on psychotropic medications and those individuals whose support plans indicate such a need.
- Ensures that communication is maintained with individuals, parents, concerned family and guardians as well all other service providers regarding all aspects of the individual.
- Participates in the admissions and discharge processes for individuals.
- Actively monitors programs and documentation of programs for service delivery and quality.
- Analyzes data and makes appropriate program interventions as needed to assist individuals progress.
- Completes Monthly, Quarterly and other assigned progress reports for all programs.
- Conducts all internal monitoring, completes monitoring reports and action plans based on findings.
- Completes corrective action assignments for internal/external monitoring in a timely manner.
- Assist with various committee duties and processes such as coordinating committee, chairing committee, taking minutes, etc. and various other meetings as needed.
- Provides leadership and supervision to assigned staff and assists with duties to ensure overall program consistency and compliance with applicable State and Federal standards, regulations, and guidelines.
- A Bachelor’s degree in Human Services or a similar related field from an approved, regionally accredited college or university required.
- One (1) year of experience working with intellectually and developmentally disabled individuals required.
- A minimum of one (1) year, previous supervisory experience.
- Valid FL Driver's License with an acceptable driving record for the past three years
- Pass a level II background including TB test & drug screening
